Job summary

Foundation Medical Partners (Affiliated with Southern New Hampshire Medical Center) seeks a Registered Nurse for the Nashua ambulatory clinic. The RN will assist providers, prep patients, take vitals, and perform triage and telehealth activities.

A NH RN license and nursing degree are required, with BLS/CPR certification. Responsibilities include administering meds, vaccines, point-of-care testing, and patient education on self-management, nutrition, and wellness as part of the care plan.

Qualifications

• Carry out the nursing process in person, by telephone and via telehealth.
• Assess and identify patient needs, plan and evaluate progress toward goals of care.
• Collaborate with Physicians, Physician Assistants and other health professionals to design a care plan.
• Administer prescribed medications, vaccines, and perform point-of-care testing.
• Educate patients on self-management, medications and wellness.

Responsibilities

• Carry out nursing process in person, by telephone and via telehealth modalities.
• Assess and identify patient needs, plan to achieve outcomes, intervene, and evaluate progress toward goals.
• Collaborate to design and implement a plan of care with the interdisciplinary team.
