Job summary

Foundation Medical Partners Inc. is seeking a Registered Nurse Ambulatory to provide professional nursing care across ambulatory practices in Nashua, NH. You will work in-person and via telephone triage/telehealth, delivering safe, evidence-based, patient-centered care.

Responsibilities include administering medications and vaccines, educating patients, and documenting care in the EMR. Clinically you'll collaborate with physicians and staff, and contribute to onboarding and quality care

Qualifications

• ASN required; BSN preferred.
• Current NH RN license (or NLC MA) and BLS certification.
• 1+ year nursing experience; ambulatory care a plus.

Responsibilities

• Carry out nursing process across ambulatory and telehealth settings.
• Collaborate with physicians and team to plan care.
• Administer medications, vaccines, and point-of-care testing.
• Educate patients on self-management, medications, nutrition, health, and wellness.
• Document care in the EMR and assist with pre-visit planning.
• Provide leadership and onboarding support to clinical staff.
• Respond to urgent situations per facility guidelines.
• Maintain licensure and ongoing educational requirements.
