Job summary

Southern New Hampshire Health in Nashua, NH is seeking a Registered Nurse – Ambulatory to deliver high-quality, patient-centered care in an outpatient setting. You will work with providers and interdisciplinary teams to support clinical operations, patient education, and care coordination across in-person, telephone triage, and telehealth encounters.

The role requires an Associate Degree in Nursing, an active RN license, BLS, and strong communication and teamwork.

Qualifications

• Graduate of an accredited nursing program with active RN license.
• Current BLS/CPR certification required.
• Ambulatory care experience a plus; EMR proficiency preferred.

Responsibilities

• Deliver professional nursing care using the nursing process across in-person, telephone, and telehealth settings.
• Collaborate with physicians and care teams to implement patient care plans.
• Educate patients on medications, disease management, wellness, and preventive care.
• Support providers during exams, procedures, and treatments.
• Administer medications, vaccines, and tests; perform point-of-care testing and specimen collection.
• Conduct pre-visit planning to ensure readiness and identify care gaps.
• Document care accurately within the electronic medical record.
• Delegate tasks and mentor clinical support staff.
• Respond to urgent situations following established protocols.
• Maintain HIPAA, safety, and infection control standards.
• Serve as a clinical resource and mentor for peers.
