Job summary

SolutionHealth in Nashua, NH is seeking a dedicated Registered Nurse to join a multidisciplinary care team. You will assist providers, triage patients, and support telehealth visits while delivering high-quality patient care in a collaborative setting.

The ideal candidate holds NH RN licensure, BLS/CPR, and prior healthcare experience. Epic experience is a plus, with a focus on patient education and coordinated care. Flexible day shifts with comprehensive benefits are offered.

Qualifications

• Current NH RN licensure required.
• BLS/CPR certification required.
• Prior healthcare experience required.
• Epic experience is a plus.

Responsibilities

• Carry out nursing process in person, by telephone, and telehealth modalities.
• Assess and identify patient needs, plan to achieve outcomes, intervene, and evaluate progress toward goals of care.
• Collaborate to design and implement a plan of care with the interdisciplinary team including Physicians, Physician Assistants, and other health professionals.
• Administer prescribed medications, treatments, and vaccines, perform point-of-care testing, and collect and handle specimens within the scope of practice as determined by the Nurse Practice Act of the appropriate state.
• Educates patient on self-management activities, medications, nutrition, health, and wellness based on the plan of care designed collaboratively with Provider(s).
