SolutionHealth in Nashua, NH is hiring an experienced Registered Nurse to support a patient-focused care team. You will room patients, take vitals, triage, and assist providers across in-person and telehealth visits, with room to grow clinically.

The role requires NH RN licensure, current BLS/CPR, and prior healthcare experience; Epic experience is a plus. We offer flexible day shifts, full-time/part-time benefits, tuition reimbursement, and a strong commitment to ongoing education and career

Qualifications

• Current NH RN licensure is required.
• BLS/CPR certification required.
• Prior work experience in a healthcare setting is required.
• Epic experience is a plus.

Responsibilities

• Carry out the nursing process in person, by telephone (telephone triage), and via telehealth modalities.
• Assess and identify patient needs, plan to achieve appropriate outcomes, intervene, and evaluate progress toward goals of care.
• Collaborate to design and implement a plan of care with the interdisciplinary team including Physicians, Physician Assistants, and other health professionals.
• Administer prescribed medications, treatments, and vaccines, perform point-of-care testing, and collect and handle specimens within the scope of practice as determined by the Nurse Practice Act of the appropriate state.
• Educates patient on self-management activities, medications, nutrition, health, and wellness based on the plan of care designed collaboratively with Provider(s).
