Job summary

Foundation Medical Partners is seeking a Registered Nurse for our ambulatory/outpatient practice in Nashua, NH. The RN will provide comprehensive nursing care, collaborate with an interdisciplinary team, and support preventive care and chronic disease management.

Responsibilities include education on medications, wellness, nutrition, and self-management, plus administering vaccines and point‑of‑care testing. Proficiency with EPIC EMR and strong patient-centered care are required.

Qualifications

• Registered Nurse license in NH, MA, or both.
• Ambulatory care nursing certification (RN-BC) or relevant specialty.
• Proof of valid Driver’s License.
• Current BLS/CPR training.
• 1 year of experience preferred.

Responsibilities

• Provide nursing care through assessment, planning, intervention, and evaluation in outpatient settings.
• Collaborate with interdisciplinary teams to implement patient care plans and education.
• Administer medications, vaccines, and point‑of‑care testing; respond to emergencies per protocols.
• Maintain EMR documentation and support quality improvement initiatives.
