Job summary

Foundation Medical Partners in Nashua, NH is seeking a Registered Nurse for ambulatory Medicine-Pediatrics. You will assist providers, room patients, take vitals, triage, and support telehealth visits.

Responsibilities include implementing care plans, administering medications and vaccines, educating patients on self-management, and collaborating with physicians, PAs, and the care team to deliver high-quality care.

Qualifications

• Must hold NH Registered Nurse licensure.
• Current BLS/CPR training required.
• Prior work experience in a healthcare setting preferred.

Responsibilities

• Carry out the nursing process in person, by telephone, and via telehealth modalities.
• Assess and identify patient needs, plan to achieve outcomes, intervene, and evaluate progress.
• Collaborate with Physicians, Physician Assistants, and other health professionals to design and implement a plan of care.
• Administer prescribed medications, treatments, vaccines, and perform point-of-care testing.
• Educate patients on self-management activities, medications, nutrition, health, and wellness.
