Foundation Medical Partners, affiliated with Southern New Hampshire Medical Center, is seeking a qualified RN to join our Immediate Care team. You will assess patients, triage, take vitals, administer medications, and collaborate with physicians and other health professionals.

A NH RN license and BLS/CPR are required; Epic experience is a plus. We offer flexible day shifts, competitive pay, and comprehensive benefits to both full-time and part-time staff.

Qualifications

• NH RN licensure required and current.
• BLS/CPR certification required.
• Experience in a healthcare setting required.
• Epic experience preferred.

Responsabilités

• Carry out nursing process in person, by telephone, and via telehealth modalities.
• Assess patient needs, plan to achieve outcomes, intervene, and evaluate progress toward goals of care.
• Collaborate to design and implement a plan of care with the interdisciplinary team including Physicians, Physician Assistants, and other health professionals.
• Administer prescribed medications, treatments, and vaccines, perform point-of-care testing, and collect and handle specimens within the scope of practice as determined by the Nurse Practice Act of the appropriate state.
• Educates patient on self-management activities, medications, nutrition, health, and wellness based on the plan of care designed collaboratively with Provider(s).
