Job summary

Qualifications

• Two years of RN experience with case management of chronic and complex conditions.
• Two years RN clinical, hospital-based experience, inpatient or outpatient setting.
• Valid California Registered Nurse License without restriction.
• Graduate of an accredited RN program.
• Experience in identifying and analyzing care/ service needs for patients.
• Knowledge of nursing care plans and ability to develop care plans utilizing evidence-based guidelines.
• Skills to perform in-depth chart analysis to provide accurate information in a concise, informative manner.
• Ability to perform work in telephonic and/or on-site setting, performing assessments, disease management, self-management education, and complex case management of high-risk patients.
• Knowledge of care/services in differing levels of care (ambulatory, institutional), community-based resources, SNFs, home health, hospice and palliative care.
• Analytical skills to identify and obtain pertinent data from multiple sources to identify patient care needs.
• Ability to make independent judgments based on clinical knowledge and experience for short and long-term goals.
• Ability to work with all levels of staff to foster collaboration.
• Effective time management to accommodate projects and deadlines.
