Job summary

You will help patients and families navigate referrals, follow-up appointments, and care transitions, bringing strong communication skills and a collaborative approach to support optimal patient outcomes.

Qualifications

• Bachelor's degree in nursing (BSN) from an accredited school.
• Licensed to practice nursing in Florida.
• Ambulatory Care Nurse Certification or AAACN Care Coordination/Transition Management certification preferred.
• 1–2 years of related clinical experience.
• Strong verbal and written communication; conflict resolution and negotiation skills.

Responsibilities

• Directs, coordinates, and monitors clinical activities in an ambulatory clinic.
• Interface with multidisciplinary care teams and clinic leadership to align with patient care goals.
• Assist patients and caregivers in navigating referrals and follow-up issues.
