Job summary

Responsibilities

• Documents care provided into the patient’s electronic medical record.
• Coordinates delivery of care within the practice setting and across health care settings.
• Administers and coordinates resources and directs clinical activities and workflow.
• Refer patients to needed services as ordered by the provider.
• Foster interdisciplinary collaboration to improve quality of care and outcomes.
• Communicate openly with patients, caregivers and other health care professionals.
• Ensure access to safe, effective, timely, efficient and equitable care.
• Mentor new staff and participate in orientation and education programs.
• Preserve patients' rights to confidentiality and privacy.
