Job summary

Henry Ford Health seeks an Advanced Pharmacy Practice Provider (APPP) to drive smarter medication use across its regional health system. You will partner with medical leaders and primary care providers to develop strategies that elevate patient care quality, improve experiences, and manage costs.

You will lead medication-use initiatives, collaborate with care teams, and mentor pharmacists while supporting population health and value-based care programs.

Qualifications

• Doctor of Pharmacy degree with advanced clinical training.
• Completion of PGY1 and PGY2 residencies; ambulatory/primary care experience.
• Licensed pharmacist in Michigan; BCACP or BCPS or eligible within 1 year.

Responsibilities

• Oversee medication use strategies and patient care within the region.
• Partner with medical and administrative leaders and providers for coordinated care.
• Identify opportunities to streamline patient care and reduce costs.
• Support population health strategies and value-based care initiatives.
• Contribute to research, quality initiatives, and formulary management.
• Mentor pharmacists on the healthcare team.
• Improve provider wellness and patient outcomes.
