Bayhealth Medical Center in Dover, DE seeks an Ambulatory Clinical Pharmacy Specialist to deliver ambulatory clinical services under CPAs and MEC credentials, helping optimize therapy and reduce costs within Bayhealth's ambulatory model.

The role collaborates with prescribers and care teams, conducts patient visits, reconciliations, and monitoring, and supports chronic disease management and specialty medication care coordination; precepts students and contributes to research and quality

Qualifications

• Education: Bachelor Degree Pharmacy.
• Licensure: Licensed Pharmacist with BPS certifications in a specialty and pharmacotherapy, ambulatory care, cardiology or geriatrics.
• Experience: Minimum one year of clinical pharmacy experience including residency.

Responsibilities

• Provide ambulatory clinical pharmacy services under CPA and MEC credentials, with assessment and documentation of therapeutic plans.
• Conduct patient visits for medication management, including reconciliation, lab monitoring, and disease state management.
• Collaborate with Bayhealth providers to implement evidence-based medication interventions to improve quality and reduce costs.
• Address referrals ensuring closed-loop communication with clinicians.
• Support access and affordability through prior authorizations and financial assistance programs.
• Provide oversight for patients on specialty meds, ensuring safe initiation and continuity of therapy.
• Educate patients, families, and healthcare teams on medications.
• Precept students, interns, or residents as assigned.
• Develop or improve systems to enhance patient care.
• Participate in research and publish related to ambulatory and specialty pharmacy.
• Contribute to policy, quality improvement, and committee work; document interventions in EHR and report outcomes.
