The Ambulatory Clinical Pharmacy Specialist provides ambulatory clinical pharmacy services. his role functions as a credentialed, privileged member of the healthcare team, practicing under collaborative practice agreements (CPAs) to optimize therapeutic outcomes, enhance medication access, and reduce the overall cost of care. The Specialist collaborates with prescribers, nurses, and multidisciplinary team members to design, implement, monitor, and document therapeutic plans that reflect safe, effective, and evidence-based medication use. These activities are recognized nationally as Comprehensive Medication Management (CMM) and serve as the foundation of Bayhealth's ambulatory pharmacy model. The Specialist serves as a clinical resource for providers, staff, and patients while supporting CMM, chronic disease management, and specialty medication care coordination.
Education: Bachelor Degree Pharmacy
Credential(s): Licensed Pharmacist, Board of Pharmacy Specialties (BPS) Certified in a specialty; Board of Pharmacy Specialties (BPS) Certified in Pharmacotherapy, Ambulatory Care, Cardiology, or Geriatrics.
Experience: Required: One year of clinical pharmacy experience, including residency experience.
