Qualifications

Responsibilities

• Provide ambulatory clinical pharmacy services under CPAs, including assessment, monitoring, and documenting therapeutic plans.
• Conduct patient visits for medication management, reconciliation, labs, counseling, and disease state management.
• Collaborate with providers to implement evidence-based interventions for quality, safety, and cost reduction.
• Address referrals with closed-loop communication to referring clinicians.
• Coordinate prior authorizations, financial assistance, and benefit investigations to support patient access.
• Oversee specialty medications for safe initiation, monitoring, and continuity of therapy.
• Educate patients, families, and healthcare staff; precept students and residents as assigned.
• Develop and improve systems/services to enhance ambulatory care.

Astrya Global, a San Diego based medical staffing agency, is hiring a PHARM for a permanent assignment in Dover, Delaware.

Responsibilities for a PHARM:

• Provide ambulatory clinical pharmacy services under pharmacy collaborative practice agreements (CPAs) as a credentialed and privileged member of the healthcare team, including assessment, monitoring, and documentation of therapeutic plans.
• Conduct patient visits for medication management, including medication reconciliation, laboratory monitoring, patient counseling, and disease state management.
• Collaborate with providers and care teams to implement evidence-based medication interventions that support quality, safety, and cost reduction goals.
• Address referrals received from providers, ensuring closed-loop communication with referring clinicians.
• Support patient access and affordability through coordination of prior authorizations, financial assistance programs, and benefit investigations.
• Provide clinical oversight for patients on specialty medications, ensuring safe initiation, monitoring, and continuity of specialty medication therapy.
• Provide drug information and education to patients, families, and healthcare team members.
• Precept pharmacy students, interns, and/or residents as assigned.
• Develop new and/or improve systems and/or services to enhance the level of care provided to patients.
• Participate in research and publications related to applicable ambulatory and specialty pharmacy practice areas.
• Participate in committee workgroups, policy development, and quality improvement initiatives that advance ambulatory pharmacy services.
• Document all interventions in the electronic health record and report outcomes consistent with national standards for ambulatory pharmacy practice.
• Perform all other duties as assigned within the scope and range of job responsibilities.
