Job summary

Logan Health in Kalispell, MT, seeks an Ambulatory Pharmacist for the Oncology Clinic to support patients on oral therapies and monitor labs for continuity of care.

The role collaborates with the specialty pharmacy team, provides pharmacotherapy recommendations, and participates in patient counseling and outcomes. A PGY2 in ambulatory/oncology is preferred; BCOP is a plus.

Qualifications

  • PharmD from an ACPE-accredited college of pharmacy.
  • Current Montana State Board of Pharmacy license.
  • Oncology Pharmacy Specialty Certification (BCOP) preferred.
  • PGY2 residency in Ambulatory Care or Oncology preferred.
  • Accurate data entry and strong math skills.
  • ACLS required; RQI applicable to assigned areas.

Responsibilities

  • Evaluates and verifies orders for therapeutic appropriateness and dosing.
  • Provides patient-specific pharmacotherapy recommendations to stakeholders.
  • Performs daily pharmacokinetic monitoring and EMR documentation.
  • Implements formulary decisions and updates CPAs per protocol.
  • Supervises trainees and precepts students on rotations.
  • Collaborates with providers and nursing staff to optimize care.

Logan Health has an opportunity for an Ambulatory Pharmacist to join their team in our Oncology Clinic!

As a key member of our pharmacy team, this position will play a vital role in supporting patients prescribed oral oncology treatments. This position ensures continuity of care by monitoring lab results, managing adverse effects, performing medication reconciliation, and providing counseling to patients. You'll also collaborate closely with our specialty pharmacy team to address medication access and availability. Additionally, you'll serve as a trusted clinical resource to providers and nursing staff.

Qualifications:
  • PharmD from a College of Pharmacy accredited by American Association of Colleges of Pharmacy, or equivalent clinical experience required.
  • Current Montana State Board of Pharmacy, Registered Pharmacist License required.
  • Oncology Pharmacy Specialty Certification (BCOP) preferred
  • Completion of PGY2 in Ambulatory Care, Oncology or related area preferred
  • Accurate data entry skills, analytical and advanced math skills required.
  • RQI required and as applicable to assigned area(s):
  • First Responder required for assignment: Clinic, Retail, Behavioral Health, IS and Administration Pharmacists.
  • ACLS required for all other assigned area(s).
Job Specific Duties:
  • Evaluates and verifies orders for therapeutic appropriateness, proper dosing, optimal dosage form, drug interactions, allergic reactions, route and rate of administration, and therapeutic redundancy.
  • Provides patient-specific pharmacotherapy recommendations and drug information to the appropriate stakeholders as applicable to assigned area(s).
  • Performs daily review of pharmacy reports including pharmacokinetic monitoring as applicable to assigned area(s).
  • Documents clinical interventions appropriately in electronic medical record.
  • Implements all drug use policy and formulary decisions of the Pharmacy and Therapeutics committee.
  • Reviews and updates Collaborative Practice Agreements per department protocol for assigned area(s).
  • Supports and precepts pharmacy students on practice rotations as applicable to assigned area(s).
  • Assesses and evaluates the medical and drug literature. Compares and contrasts therapeutic regimens and uses for new or existing drug treatments.
  • Provides direct and indirect patient care for patients through Collaborative Practice Agreements as applicable to assigned area(s).
  • Develops and participates in operational and clinical projects as determined by departmental needs and ongoing projects as applicable to assigned area(s).
  • Performs direct patient care rounding, order verification, discharge counseling, and education of other healthcare professionals as applicable to assigned area(s).
  • Supervises the pharmacy technicians as applicable to assigned area(s). Serves as a preceptor for pharmacy students on rotations as applicable to assigned area(s).
  • Maintains pharmacy knowledge through continuing education and applies new principles to current practice as needed.
  • Exhibits knowledge of policies and procedures as they relate to job duties and HIPPA.
  • Coordinates medication ordering, preparation, dispensing, and administration systems for the oncology patient populations as applicable
