Job summary

Akido Labs, Inc. seeks a Clinical Pharmacist to provide ambulatory pharmaceutical care within the Rhode Island Primary Care Physicians Corporation (RIPCPC) ACO.

You will integrate with primary care practices, focusing on COPD population health, transitions of care, inhaler education, and adherence support to optimize regimens and reduce avoidable healthcare utilization. You will collaborate with providers and care management teams to deliver evidence-based care, document encounters in the EHR,

Qualifications

• Advanced training in chronic disease management relevant to the role.
• PharmD or BS in Pharmacy with required professional experience.
• Licensure in Rhode Island and adherence to regulatory standards.

Responsibilities

• Serve as medication expert on the interdisciplinary care team.
• Reconcile and optimize chronic disease regimens (esp. COPD).
• Deliver patient-centered medication therapy recommendations with care teams.
