Job summary

Akido Labs, Inc. in Rhode Island seeks a Clinical Pharmacist for Ambulatory Care within a COPD population health program.

The pharmacist will be embedded in primary care practices, delivering evidence‑based, patient‑centered medication management to optimize regimens and improve chronic disease outcomes. Responsibilities include COPD guideline‑directed therapy, inhaler education, transitions of care, and collaboration with an interdisciplinary team to reduce avoidable healthcare utilization.

Qualifications

• Pharmacist must be licensed in Rhode Island and work under CPA rules.
• Advanced training or residency in ambulatory care or chronic disease management preferred.
• Board certification or ASHP residency in area of practice is preferred.
• Pharmacist should be able to operate at top of license and make independent clinical decisions.

Responsibilities

• Serve as medication expert on interdisciplinary care team.
• Reconcile regimens for COPD patients and optimize therapy.
• Provide COPD guideline-directed therapy and inhaler education.
• Engage high-risk COPD patients identified through population health tools.
• Document encounters in the EHR and report program outcomes.
