The role focuses on medication optimization, inhaler education, adherence support, and transitions of care, with broad ambulatory pharmacy services and quality initiatives. You will ensure safe, evidence-based prescribing, document encounters in the EHR, and contribute

Competenze

• Prior experience practicing under a Collaborative Practice Agreement (CPA).
• Comfortable making independent clinical decisions without requiring physician sign-off at every step.
• Unrestricted pharmacist licensure in Rhode Island.
• Advanced training or experience in chronic disease management (Board of Pharmaceutical Specialties cert or ASHP residency).
• Pharmacist education and years of professional experience as stated.

Mansioni

• Functions as the medication expert on the interdisciplinary care team.
• Reconciles and assesses medication regimens for adherence, safety, and optimization of therapy.
• Provides evidence-based, patient-centered medication therapy recommendations with providers.
• Delivers COPD-focused state management, inhaler education, and adherence monitoring.
• Coordinates transitions of care and follow-up with primary care teams.
