Job summary

Rhode Island Primary Care Physician’s Corporation (RIPCPC) is seeking a Clinical Pharmacist to deliver ambulatory care services within a COPD population health program, embedded in primary care practices.

You will manage COPD patients, optimize medications, provide inhaler education, and collaborate with the care team to improve outcomes while supporting quality initiatives and transitions of care.

Qualifications

• Board certification by BPS or ASHP-accredited residency in the area of practice.
• PharmD degree with two years' professional experience.
• Bachelor of Science in Pharmacy with three years' professional experience.
• Licensed pharmacist in Rhode Island in good standing.

Responsibilities

• Provide ambulatory pharmaceutical services within the COPD population health program.
• Collaborate with interdisciplinary team to optimize regimens and outcomes.
• Educate patients on COPD management and inhaler technique.
• Document encounters in the EHR and report program metrics.
