Job summary

Rhode Island Primary Care Physicians Corporation (RIPCPC) seeks a Clinical Pharmacist embedded in primary care practices to lead ambulatory pharmacy services, focusing on COPD patient populations and transitions of care. You will work with an interdisciplinary team to optimize medications, reduce hospital utilization, and support quality initiatives.

The role requires CPA experience, RI licensure, and a PharmD with 2 years (or BS in Pharmacy with 3 years).

Qualifications

  • Experience under a Collaborative Practice Agreement (CPA) enabling independent medication management.
  • Ability to make independent clinical decisions within licensure.
  • Active pharmacist license in Rhode Island.
  • Board certification in area of practice or ASHP residency preferred; otherwise CE certificate acceptable.
  • PharmD required with 2 years of professional experience; BS in Pharmacy with 3 years of experience also acceptable.

Responsibilities

  • Serve as medication expert on the interdisciplinary care team.
  • Reconcile and optimize medication regimens for safety and efficacy.
  • Provide evidence-based, patient-centered therapy recommendations.
  • Deliver COPD-focused care including inhaler education and adherence monitoring.
  • Identify high-risk COPD patients via population health platforms and coordinate care.
  • Document encounters in the electronic health record and report outcomes.
