Zusammenfassung

Northwell Health is seeking a dedicated Registered Nurse to provide age-specific care using the nursing process and to collaborate with physicians and the health team in coordinating patient care. The role emphasizes leadership, clinical judgment, and directing activities on the patient care unit.

The Nurse will assess physiologic and psychosocial health, develop patient outcomes, implement plans, document care, and participate in discharge planning and education for patients and families.

Qualifikationen

• Graduate from an accredited School of Nursing.
• Must be enrolled in BSN within two (2) years and obtain a BSN Degree within five (5) years of job entry date.
• Current license to practice as a Registered Professional Nurse in New York State required, plus specialized certifications as needed.

Aufgaben

• Assess the patient’s physiologic health status and document plan of care.
• Assess psychosocial health status and identify needs for support services.
• Identify patient outcomes and establish individualized goals.
• Formulate and prioritize the plan of care in coordination with the health team.
• Implement and evaluate the plan of care, ensuring safety and timely administration of treatments.
• Evaluate care outcomes and contribute to performance improvement initiatives.
• Collaborate with care team and physicians to adjust treatments and coordinate referrals.
• Provide wound care and pressure ulcer management according to procedures.
• Administer medications safely and educate patients and families about drugs and interactions.
• Participate in patient and family education, discharge planning, and follow-up resources.
• Perform related duties as required; maintain essential documentation according to policy.
