Job summary

Sibley Memorial Hospital in Washington, DC, seeks a Clinical Pharmacy Specialist - Ambulatory Oncology to deliver advanced pharmaceutical care and medication management with autonomy and clinical judgment. You will lead complex patient care activities and support evidence-based, cost-effective therapy.

Collaborate with prescribers and a multidisciplinary team to optimize outcomes, participate in committees and research, and educate patients, students, residents, and other healthcare

Qualifications

• D doctor of pharmacy degree (PharmD) or equivalent.
• PGY1/PGY2 residency or equivalent experience required.
• 5+ years in clinical pharmacy practice preferred.

Responsibilities

• Provide advanced pharmaceutical care and comprehensive medication management for ambulatory oncology patients.
• Lead and participate in quality-improvement initiatives and guideline development.
• Collaborate with prescribers and multidisciplinary teams to optimize therapy.
• Educate patients, students, residents, pharmacists and other healthcare professionals.
