Job summary
Prime Healthcare Olympia Fields Hospital is seeking an Ambulatory Clinical Pharmacist in the Ambulatory Pharmacy department. The role focuses on direct patient care in outpatient settings, including HOPD pharmacist clinics, specialty pharmacy, and population health services, with collaboration with prescribers in ambulatory settings.
The position involves working with oncology/infusion/chemotherapy drugs, documenting interactions in EMR, and participating in quality initiatives.
Qualifications
- Current state Pharmacist license is required.
- PharmD or B.S. in Pharmacy is required.
- PGY-1 Pharmacy Practice Residency is required (significant experience may be considered).
- Board certification (BCPS, BCACP, or equivalent) within 18 months of hire is required.
- Strong verbal and written communication skills are required.
- Strong customer service skills are required.
Responsibilities
- Provide patient care and advanced clinical pharmacy services in ambulatory pharmacy settings.
- Collaborate with prescribers in ambulatory settings and manage medication therapy.
- Document patient interactions across EMR platforms and communicate identified issues and solutions.
