Sibley Memorial Hospital, part of Johns Hopkins Medicine, seeks a Clinical Pharmacy Specialist - Ambulatory Oncology to provide advanced pharmaceutical care and medication management. You will lead complex patient care activities, support guidelines, and collaborate with prescribers to optimize outcomes.

Expect full-time day shifts, with on-call occasional. Requires PGY1 and PGY2 residencies or equivalent, 5+ years of clinical pharmacy experience and licensure; board certification within 2 years

Qualifications

• Graduation from an accredited College of Pharmacy resulting in a B.S. or PharmD degree
• Completion of a PGY1 ASHP-accredited residency or equivalent experience (3 years general practice) is required
• Completion of a PGY2 ASHP-accredited residency or equivalent experience (3 years specialized practice) is required
• Board certification or eligibility and active pharmacist licensure within state are required

Responsibilities

• Provide advanced pharmaceutical care and comprehensive medication management for ambulatory oncology patients.
• Lead complex patient care activities and support clinical guidelines and policy development.
• Collaborate with prescribers and the healthcare team to optimize outcomes and participate in quality-improvement initiatives.

Job Summary:

The Clinical Pharmacy Specialist - Ambulatory Oncology, Provides advanced, evidenced-based pharmaceutical care and comprehensive medication management to ensure the safe and effective use of medications. Leads complex patient care activities, supports clinical guidelines and policy development, and contributes to quality-improvement initiatives that enhance safety and
outcomes. Functions with a high degree of autonomy and clinical judgment. Works collaboratively with prescribers and the multidisciplinary healthcare team to optimize patient outcomes and support evidence-based, cost-effective therapy. Participates in multidisciplinary committees and research initiatives. Provides education to patients, students, residents, pharmacists, and other healthcare practitioners.

Minimum Education and Experience:

• Graduation from an accredited College of Pharmacy resulting in B.S. or Pharm D. degree
  Completion of a post-graduate year 1 (PGY1) ASHP accredited residency program or equivalent experience (3
  years general practice) required
  Completion of a post-graduate year 2 (PGY2) ASHP accredited residency program or equivalent experience (3
  years specialized practice) required
  5+ years of experience in clinical pharmacy practice requiredCompletion of an ASHP accredited Post-Graduate Year 1 Pharmacy Residency is equivalent to 3 years of
  practice experience; completion of an ASHP accredited Post-Graduate Year 2 Pharmacy Residency is
  equivalent to an additional 3 years of practice experience (6 years of experience total for PGY1 and PGY2
  training)
  Pharmacist licensure in applicable state.
  Board Certification in applicable area required within 2 years of employment.
  BLS, ACLS and/or PALS may be required based on practice setting.

Shift:

Full-Time, Day Shift, Monday - Friday 7:30am - 4pm; Ocassional On-call work required.
