Job summary

TriHealth is seeking a Nurse Navigator to guide patients through their healthcare journey, coordinating care from diagnosis through recovery. You will work with physicians and care teams to ensure timely, appropriate care and empower patients in self-management.

The role emphasizes education, communication, and collaboration within a multidisciplinary team in a supporting, patient-centered culture.

Qualifications

• Associate's Degree in Nursing required.
• 2–3 years' clinical nursing experience in the specialty area for Navigator role required.
• 2–3 years' home care nursing experience preferred.

Responsibilities

• Assesses patients and caregivers activations, literacy, and self-management.
• Evaluates patient functional abilities and determines need for intervention.
• Evaluates medical necessity and optimizes patient outcomes across care continuum.
• Serves as liaison for referrals to physicians and community resources.
• Supports data collection and reporting for value-based contracts.
• Develops plans of care, documents progress, and facilitates resource utilization.
