Job summary

Atlantic Health is seeking a registered nurse to join our ambulatory care team in New Jersey. The role emphasizes comprehensive patient assessment, care coordination across the health care continuum, and education for patients and families.

You will document in Epic Ambulatory, maintain patient privacy under HIPAA, and collaborate with physicians and staff to Close care gaps. The ideal candidate has a valid RN license in New Jersey, a BLS credential, at least 2 years of ambulatory experience,

Qualifications

• High School Diploma or GED required; nursing program graduate.
• Bachelor's degree in nursing preferred.
• At least 2 years of ambulatory nursing experience.
• RN license valid in New Jersey.
• BLS certification required.
• Experience with EPIC Ambulatory EMR.

Responsibilities

• Perform nursing process including assessment, planning, and evaluation.
• Coordinate patient assessment and ongoing care to improve outcomes.
• Provide patient and family teaching and education.
• Maintain patient privacy and confidentiality per HIPAA.
• Document communications and care in Epic EMR.
• Triages patient calls and supports scheduling.
