• Associates degree in Nursing from an accredited program.

Responsibilities

• Using the nursing process, provides ongoing assessment of the nursing care needs of a defined group of patients. Develops a written plan of care and assists patients and families in achieving health care goals.
• Coordinate and facilitate health care services for patients across the care delivery system with the health care team.
• Oversee and participate in patient assessment, care planning, and implementation of nursing care.
• Provide patient and family education, and offer telephone consults as part of ongoing care.
• Ensure safe environment, infection control, and adherence to standards and policies.
