Job summary

The University of Kansas Health System is seeking an Ambulatory Clinic RN Care Coordinator (RN) for the Landon Center on Aging; Neurology. This full-time role supports physicians during clinic sessions, coordinates patient care, and ensures the plan of care is communicated clearly to patients and families.

The RN will triage, manage patient flow, document in the EMR, provide health education, and liaise with front desk staff, nursing and clinic leadership to optimize outcomes and safety.

Qualifications

• High School Graduate
• Bachelor Degree Nursing
• 2 or more years of RN experience.

Responsibilities

• Manage patient flow, triage, and direct patient care.
• Liaison between front desk staff, nursing staff, physicians, and clinic manager.
• Develop patient outcomes and coordinate care with providers.
• Provide nursing support to optimize patient outcomes and education services.
• Coordinate continuity of care with patients and families post-discharge/ER.
• Document assessments and plans in EMR; maintain patient confidentiality.
