Job summary

Northwell is seeking a Registered Nurse to provide age-specific nursing care using the nursing process. The role collaborates with physicians and the health team to coordinate procedures and treatments on a patient-care unit.

Leadership and clinical judgment are employed to direct care and delegate activities. The nurse will assess physiologic and psychosocial health, identify outcomes, plan and implement care, and provide discharge planning and education to patients and families.

Qualifications

• Graduate from an accredited School of Nursing.
• Bachelor’s Degree in Nursing, preferred.
• Current NYS RN license required.
• Experience in ambulatory Internal Medicine/Endocrinology/Pulmonary preferred.
• Bilingual in Spanish preferred.

Responsibilities

• Assess patient physiologic health status and document plan of care.
• Assess patient psychosocial health status and support needs.
• Identify patient outcomes and establish goals.
• Formulate and document an effective plan of care.
• Implement and monitor the plan with safety and efficiency.
• Evaluate care effectiveness and outcomes.
• Collaborate with care team and delegate tasks as needed.
• Provide wound care and pressure ulcer management.
• Administer medications correctly and safely.
• Educate patient and family; coordinate discharge and follow-up care.
• Perform related duties as required per department policy.
