Job summary

Commonwealth of Virginia, Culpeper region, is hiring a Licensed Practical Nurse (Ambulatory) to provide direct patient care across multiple clinics including SSDC and the Float Team. The role will split time between in-person care in Culpeper and remote work-from-home support when needed, with training provided to cover primary, specialty, and pediatric care in the Culpeper area.

Qualifications

• Graduated from an accredited Licensed Practical Nurse program.
• Experience: None required.
• Licensed to practice as an LPN in Virginia prior to onboarding; BLS certification required.

Responsibilities

• Assist the RN/provider with data collection on the physical, psychological, social, and cultural dimensions of patients.
• Organize and prioritize nursing care activities with the interdisciplinary team.
• Implement age-appropriate interventions based on patient’s needs as directed by the provider.
• Assist in evaluating the effectiveness of care and progress toward outcomes.
• Provide ongoing education and reinforcement of care plans developed by the Provider/RN.
• Provide a safe environment and safe delivery of care.
• Assumes responsibility for professional development of self and contributes to others.
• Demonstrates teamwork in the delivery of patient care.
