Qualifications

Responsibilities

• Train clinical staff on Partnership for Health and medication adherence interventions
• Provide patient education on prevention, risk reduction, sexual health, and substance use
• Coordinate follow-up appointments and liaise with providers
• Track patient data and monitor intervention fidelity in EMR
• Lead daily huddles and coordinate outreach for chronic health maintenance
