Qualifications

• Graduate of an accredited nursing program with an Associate’s or BSN.
• BSN preferred; if no BSN, must enroll in a program within one year and complete BSN within four years.

Responsibilities

• Apply nursing process for patient care across the care continuum.
• Assess patients and families; plan, implement, and evaluate care.
• Educate patients and families about illness, procedures, and plan of care.
• Document interventions per policy and communicate with care team.
• Delegate appropriately and ensure task completion by staff.
• Provide follow-up care via home visits or phone when needed.
