Job summary

Foundation Medical Partners in New Hampshire is seeking a Registered Nurse for its clinical float pool. Floats to multiple practice sites provide interim coverage, under supervision of Providers and the Director of Nursing, performing essential ambulatory care nursing functions.

You will assess, plan, implement, and evaluate care; collaborate with physicians, APRNs, and allied staff; administer medications and vaccines, perform point-of-care testing, educate patients, and document in the medical

Qualifications

• Registered Nurse with license in the state of practice (NH, NLC state, MA, or both).
• Certification in Ambulatory Care Nursing (RN-BC), CCTM, or relevant clinical specialty.
• Associate’s in Nursing required
• BSN Preferred
• Proof of valid Driver’s License.
• Current BLS/CPR training.
• 1 year of experience preferred

Responsibilities

• Carries out the nursing process in-person, by telephone and via telehealth modalities.
• Collaborates to design and implement a plan of care with the interdisciplinary team.
• Administers prescribed medications, treatments, and vaccines; performs point-of-care testing; collects specimens.
• Educates patient on self-management activities and plan of care.
• Documents the nursing process in the medical record with accuracy and detail
