Job summary

Shields Health Solutions is hiring an Ambulatory Clinical Pharmacy Specialist to elevate patient care at Providence Regional Medical Center Everett in Everett, WA. You will conduct in-person and virtual patient assessments, optimize therapy, and resolve medication-related problems within a CPA framework.

The role requires PharmD with Washington licensure, PGY2 ambulatory care residency or PGY1 with ambulatory experience under CPA, and at least 3 years in ambulatory practice.

Qualifications

• PharmD required and licensed to practice in Washington.
• Experience in ambulatory care and medication management recommended.
• Strong communication and collaboration with care teams essential.

Responsibilities

• Provide medication management, reconciliation, and dose optimization.
• Collaborate with physicians and care teams under a CPA.
• Monitor therapy effectiveness and safety, intervene as needed.
• Support quality-improvement and REMS initiatives.
• Assist with complex prior authorizations and workflow improvements.
