Shields Health Solutions is seeking an Ambulatory Clinical Pharmacy Specialist to join a CPA-enabled team at Providence Regional Medical Center Everett in Everett, WA. You will deliver comprehensive medication management and collaborate with care teams to optimize therapy and resolve medication-related problems.

Qualifications include PharmD, WA Pharmacist license, and either PGY2 Ambulatory Care or PGY1 with ambulatory experience under a CPA, or 3+ years of ambulatory practice.

Qualifications

• Hold a Doctor of Pharmacy (PharmD).
• Must be a licensed pharmacist in Washington.
• PGY2 Ambulatory Care Residency or PGY1+ambulatory with CPA, or ≥3 years ambulatory practice.

Responsibilities

• Provide comprehensive medication management and reconciliation.
• Collaborate with physicians and care teams under CPA to plan care.
• Monitor therapy, resolve medication-related problems, and support workflows.
