Shields Health Solutions is hiring an Ambulatory Clinical Pharmacy Specialist to elevate patient care at Providence Medford Medical Center. Onsite clinic work with CPA, collaborative practice, and in‑person/virtual assessments. Requires PharmD and Oregon licensure.

Role involves medication management, therapy optimization, and close team collaboration to improve patient outcomes. You’ll join a nationally recognized academic medical center with strong benefits.

Qualifications

• Holds PharmD degree and is licensed to practice in Oregon.
• Completes PGY2 Ambulatory Care or PGY1 with ambulatory CPA, or 3+ years ambulatory practice.
• Strong communication and organizational abilities needed.

Responsibilities

• Provide comprehensive medication management aligned with evidence‑based standards.
• Collaborate with physicians and care teams under a CPA to plan care.
• Monitor therapy effectiveness and safety; identify opportunities for optimization.
• Support operations with prior authorizations, workflow improvements, and staff education.
