Job summary

UMass Memorial Health in Worcester, MA is seeking a Clinical Pharmacist for the Outpatient Pharmacy – Prescription Center on a per diem, part-time basis. The position is on-site at Memorial Campus, with a rotating schedule and potential signing bonus, contingent on interview.

You will participate in drug therapy management, prepare and dispense medications, counsel patients, collaborate with physicians, assist with access to therapies, and provide immunizations where legally authorized.

Qualifications

• Doctor of Pharmacy (Pharm.D) or BSPharm degree and MA licensure.
• Minimum 3 years ambulatory pharmacy experience.
• Experience promoting immunizations and patient-centered care.

Responsibilities

• Participates in drug therapy management and documents care in medical records.
• Prepares, dispenses, and labels medications; reviews physician orders.
• Collaborates with physicians to optimize therapy and resolve issues.
• Assists patients with access to medications and payer navigation when applicable.
• Counsels patients and reports adverse drug events; provides immunizations where allowed.
