Qualifications

Responsibilities

• Manage patient flow, triage and direct nursing care in ambulatory clinic.
• Coordinate communications among staff, physicians and clinic manager.
• Educate patients and families; support self-management and care plans.
• Document assessments, plans and progress in EMR; relay results to physicians.
• Coordinate continuity of care across admission, discharge and ER visits.
