Job summary

Sentara Health Plans is seeking a Licensed Practical Nurse (LPN) to join a physician practice in Suffolk, VA. The LPN will support providers, room patients, administer medications and immunizations, collect specimens, and document care in the EHR.

The role emphasizes patient education, patient flow support, and collaboration with the care team to ensure safe outpatient care in a busy clinic setting.

Qualifications

• Completion of an accredited practical nursing program leading to LPN/LVN eligibility.
• A high school diploma or equivalent.
• Experience in an ambulatory or physician practice setting preferred.

Responsibilities

• Rooms patients, measures vital signs, and records health information in the EHR.
• Administers medications and immunizations within the LPN scope.
• Collects blood specimens and performs simple diagnostic tests as ordered.
• Prepares patients and treatment rooms and assists with examinations and procedures.
• Provides patient education and follow-up instructions within scope.
• Supports patient flow and processes messages and refill requests under provider direction.
• Directs the day-to-day work of medical assistants and aides as assigned.
