Job summary

Christiana Care Health System is seeking a Practice Transformation Liaison to partner with primary care and ambulatory practices to improve workflows and strengthen quality outcomes. You’ll work with providers, nurses, practice leaders, and front-office teams to identify opportunities and implement practical solutions that improve performance and patient experience.

Qualifications

• Bachelor’s degree required; master’s preferred.
• Knowledge of ambulatory/clinical practice operations and EMRs.

Responsibilities

• Build trusted partnerships with providers, practice leaders, and clinical/administrative teams across our ambulatory network.
• Assess and improve practice operations by evaluating workflows and implementing solutions.
• Advance value-based care by helping practices improve preventive care, chronic disease management, and quality outcomes.
• Lead and support change by educating practice teams and using performance data to identify opportunities.
