Qualifications

• Specialized Diploma from an accredited nursing program.
• Current RN license to practice in Pennsylvania.

• Ambulatory care experience is preferred.

Responsibilities

• Provide evidence-based nursing interventions during visits and calls.
• Collaborate with clinic care providers to coordinate patient care.
• Educate patients and families on preventive and chronic care management.
• Document all patient interactions in the EMR per standards and regulations.
• Support care coordination for abnormal results and vaccinations.
• Participate in team huddles and quality improvement activities.
