Job summary

Adelante Healthcare is seeking an experienced Registered Nurse to join the Care Delivery Integration team in Phoenix. You will collaborate with providers and staff to coordinate patient-centered care and improve outcomes, while mentoring Medical Assistants and supporting vaccine programs.

The role emphasizes clinical oversight, documentation accuracy, and adherence to quality and regulatory standards across clinic sites.

Qualifications

• Bachelor’s degree from accredited university or foreign equivalent.
• Arizona State Board of Nursing license in good standing.
• Minimum two years of ambulatory or acute care nursing experience.
• Experience with Electronic Health Records.

Responsibilities

• Direct patient care: nursing assessments, triage, medications, immunizations and education.
• Coordinate care for high-risk patients and those with chronic conditions.
• Provide clinical oversight and mentoring to Medical Assistants and clinical staff.
• Support vaccine program oversight and regulatory readiness.
