Job summary
Mountain Park Health Center in Phoenix, AZ is seeking a Registered Nurse to collaborate with providers and the care team to deliver safe, efficient, and effective care to empaneled patients. Under clinical direction, the RN facilitates health and disease management, uses evidence-based care to improve outcomes, and assists in transitions of care.
The RN will design, implement, and evaluate educational programs for nursing and support staff, act as a clinical resource, and promote outreach
Qualifications
- Registered Nurse with an associate degree in Nursing or related field.
- At least two years of professional experience.
- AZ Registered Nurse License in good standing.
- Current CPR certificate.
Responsibilities
- Performs short evaluation of patient situation to estimate severity of illness.
- Triage and direct high-risk patients to ED when needed.
- Maintain follow-up of patients directed to ED.
- Acts as a clinical resource for support staff and leadership in the department.
- Designs and evaluates educational programs for nursing staff.
- Supports outreach programs to health care professionals.
