Job summary

AltaMed is seeking a Clinical Pharmacist (PACE) to work with the healthcare team and patients, delivering ambulatory pharmacy services and safe dispensing. The pharmacist will manage patient drug therapy and provide education to patients in collaboration with other health professionals.

The role requires a California pharmacist license or FPGEC, with BCACP preferred and 2–3 years of practice. Bilingual English/Spanish/Mandarin/Cantonese strongly preferred.

Qualifications

• Pharmacist with Doctor of Pharmacy degree and California license or FPGEC.
• BCACP ambulatory care certification preferred or residency completed.
• Minimum 2 years’ pharmacist experience; 3+ years preferred.
• Bilingual English/Spanish/Mandarin/Cantonese preferred.

Responsibilities

• Provide ambulatory pharmaceutical care and safe dispensing in collaboration with the healthcare team.
• Manage patient drug therapy and provide patient education per guidelines.
