Job summary

AltaMed Health Services in Huntington Park, CA is seeking a Clinical Pharmacist for the PACE program to deliver ambulatory pharmacy services in collaboration with the healthcare team. You will manage patient drug therapy, provide education, and ensure safe medication use within a team-focused setting.

The role requires California pharmacist licensure, PharmD, and strong communication in English; bilingual skills are strongly preferred.

Qualifications

• Graduate of an ACPE-accredited college of pharmacy with a Doctor of Pharmacy degree and a California pharmacist license required, or FPGEC credential.
• Board certification (BCACP) in ambulatory care or completion of a Pharmacy practice residency preferred.
• Minimum of 2 years as a practicing Pharmacist; 3 years preferred.

Responsibilities

• Provide ambulatory pharmaceutical care services in collaboration with the healthcare team.
• Manage patient drug therapy and provide patient education to ensure safe medication use.
• Dispenser and pharmacist support for the PACE program in an in-person setting.
