AltaMed Health Services Corp. seeks a Clinical Pharmacist (PACE) to deliver ambulatory pharmaceutical care with the healthcare team, including patient education and medication management. The role supports safe dispensing and collaborative care for diverse patients in California.

Requirements include PharmD, CA license or FPGEC, and 2–3 years of pharmacy practice; bilingual English/Spanish/Mandarin/Cantonese is strongly preferred. Competitive compensation and benefits apply.

Qualifications

• Graduate of ACPE-accredited college of pharmacy with a PharmD and CA pharmacist license required, or FPGEC with Board certification.
• BCACP in ambulatory care or residency completion preferred.
• Minimum of 2 years as a practicing Pharmacist required; 3 years preferred.
• Bilingual: English/Spanish/Mandarin/Cantonese strongly preferred.
• A valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Work with healthcare team to provide ambulatory pharmacy services.
• Dispense medications and manage patient drug therapy.
• Provide patient education and collaborate with other health professionals.
