Job summary

AltaMed Health Services Corp. in California seeks a Clinical Pharmacist (PACE) to work with the healthcare team and patients to provide safe and effective pharmacy services, including ambulatory care and dispensing of medications.

The pharmacist will apply approved guidelines in collaboration with other health professionals to manage patient drug therapy and provide education. The role requires a PharmD, CA license, and preferred BCACP; bilingual skills and a strong commitment to patient care

Qualifications

• ACPE-accredited college of pharmacy graduate with PharmD and CA license.
• BCACP or residency in ambulatory care preferred.
• Minimum 2 years as a practicing pharmacist; 3+ preferred.
• Bilingual English/Spanish/Mandarin/Cantonese preferred.
• Valid BLS certification or higher per AHA/Red Cross guidelines.

Responsibilities

• Provide ambulatory pharmaceutical care services and manage patient drug therapy.
• Collaborate with the healthcare team to ensure safe and effective medication use.

Job Overview

The Clinical Pharmacist (PACE) position will work directly with the healthcare team and patients to provide safe and effective pharmacy services in addition to the dispensing of medications. The pharmacist will provide ambulatory pharmaceutical care services using approved guidelines in collaboration with other health professionals, including managing patient drug therapy and providing patient education.

Minimum Requirements

• Graduate of an ACPE-accredited college of pharmacy with a Doctor of Pharmacy degree and a California pharmacist license required, or be certified by the Foreign Pharmacy Graduate Equivalency Committee (FPGEC) issued by the National Association of Boards of Pharmacy
• Board certification (BCACP) in ambulatory care or completion of a Pharmacy practice residency is preferred.
• Minimum of 2 years as a practicing Pharmacist is required, 3 years is strongly preferred.
• Bilingual: English/Spanish/Mandarin/Cantonese, strongly preferred
• A minimum requirement of a valid BLS certification or higher, following the American Heart Association (AHA) or the American Red Cross guidelines.

Compensation

$123,607.91 - $154,509.89 annually
