Job summary

St. Luke's Health System in Twin Falls, Idaho seeks an Ambulatory Care Pharmacist for the Medication Management Clinic. The role includes independently managing chronic disease, supporting providers and patients, and collaborating with a multidisciplinary team.

Hours are Monday–Friday 8 am–5 pm with a fast-paced clinic schedule (12–20 appointments). Ideal candidates hold an ambulatory care residency (PGY2) or equivalent, with BCPS/BCACP/BCGP certification.

Qualifications

• Completion of a PGY2 Ambulatory Care Residency, completion of a PGY1 Pharmacy Residency with strong ambulatory care focus, or equivalent ambulatory care experience.
• Current Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP).

Responsibilities

• Independent clinic-based pharmacist with broad scope of practice.
• Manage a fast-paced clinic schedule of 12-20 patient appointments per day.
• Provide drug information, resolve pharmacy-related issues, and educate patients.
• Work within a multidisciplinary team with physicians, APPs, dieticians and nurses.
• Collaborate with clinical pharmacy technicians and precept pharmacy students/residents.
