Job summary

St. Luke’s Ambulatory Care Pharmacist position in Boise, ID, focuses on medication management within the Internal Medicine Clinic. You will independently manage chronic disease states and collaborate with primary care providers to optimize anticoagulation and GLP-1 therapies.

You will work with a multidisciplinary team in a clinic-based setting, with opportunities to precept students and residents. Normal hours are Monday–Friday, 8 am to 5 pm, in a supportive hospital system.

Qualifications

• Completion of a PGY2 Ambulatory Care Residency, completion of a PGY1 Pharmacy Residency with strong ambulatory care focus, or equivalent ambulatory care experience.
• Board Certification in Pharmacotherapy (BCPS), Ambulatory Care Pharmacy (BCACP), or Geriatric Pharmacy (BCGP)
• Experience with complex diabetes and insulin pump management strongly preferred
• Previous experience with the clinical management of anticoagulation or GLP-1 therapy preferred

Responsibilities

• Independent clinic-based pharmacist practice with broad scope of practice.
• Management of a fast-paced clinic schedule of 12–20 patient appointments per day.
• Delivering comprehensive support to providers and patients by answering drug information inquiries, resolving pharmacy-related issues, and providing medication education.
• Working as part of a multidisciplinary team with physicians, advanced practice practitioners, dieticians and nurses.
• Collaboration with clinical pharmacy technicians.
• Opportunities for precepting pharmacy students and residents.
• Hours: Monday–Friday 8 am to 5 pm
